To renew ICICI life insurance, you would need to submit a cheque for "ICICI Prulife Policy Number " 'A/c Payee' and write your name, contact number and policy number behind the check and can drop this cheque in the drop box.

1. Saral form (Also known as Form 2D) - Qty 2 + 1 (if submitted thro' consultant)
2. Pan card xerox
3. Form 16 original
